The weight-loss treatment market · deep dive

Step 03 · Perception

What the market believes — the regain moat, in patients' own words

79,000 evidence snippets from reviews, video, forums and articles, scored by molecule and decision axis. Every molecule wins on efficacy; the divergence — and the switching decision — lives in weight regain after stopping.

Perception here is not a survey — it is 79,000 evidence snippets extracted from what patients, clinicians and reviewers actually wrote and said, each carrying sentiment, an emotional register and a voice (first-person patient vs professional). Quotes below are verbatim from the corpus.

The leaderboard that matters

Sentiment by decision axis (−1 to +1), the axes patients actually decide on:

Trust & safety+0.44+0.39+0.16+0.16

Everyone wins on efficacy — that argument is over. The moat is regain: the tirzepatide family owns a positive "keeping it off" conversation while the semaglutide family carries the regain anxiety. That, and near-neutral tolerability, is Mounjaro's perception advantage — worth more than any spec.

One molecule breaks the pattern: retatrutide, not yet launched, is the only one whose dominant emotional register is aspiration (30% of its mentions) — the market is pre-selling itself the next upgrade.

In their own words

Patients — the results that pull people in:

"Now, here I am 76 pounds down and looking and feeling so much better." — Reddit "FIFTY POUNDS in 8 months." — Reddit

Patients — the friction that pushes back:

"After answering a few dozen health-related questions online, I was approved for Ozempic. At no point did I speak on the phone to anyone." — YouTube

Clinicians — conviction and caution in the same corpus:

"We are in the beginning of the most effective medical intervention for obesity that we've ever seen in our lives." — YouTube (physician) "I've seen my patients getting ischemic optic neuropathies after starting these meds." — Reddit (medical professional)

The hard edges:

"Do not use Wegovy if you or any of your family have ever had a type of thyroid cancer called medullary thyroid carcinoma (MTC)…" — official labelling, quoted in the corpus "Counterfeit Ozempic has been found to contain insulin instead of semaglutide." — web article

The diabetes thread

These are diabetes drugs first — semaglutide (Ozempic, Rybelsus) and tirzepatide (Mounjaro) are licensed for type-2 diabetes, re-licensed for weight loss. The crossover is live in the evidence ("I started MJ for T2D and weight loss"), it drove the 2023 diabetic-supply shortages, and it imports the insulin/sulfonylurea interaction and hypoglycaemia risks into the weight-loss conversation. Content that ignores the dual indication misreads the audience.
